    Community and Building Use Form- Community groups and organizations can submit the attached form to the Buildings and Grounds Department to utilize our buildings and facilities.   Proper Insurance requirements must be met for outside groups and organizations and in some instances charges for the use of facilities are necessary.  Please consult with the B&G Department if you have any facility use questions.  Building Use Application

    Asbestos Management: Asbestos Surveillance Completed: The federal Asbestos Hazard Emergency Response Act requires every school district to conduct a surveillance of any asbestos-containing building material in its facilities and to note its condition. This surveillance must be conducted every six months. Upon completion of the surveillance activity, school districts are required to notify all persons who use the district’s facilities.     Availability of the Asbestos Management Plan: Part 763.93 (G) (4) of the Environmental Protection Agency (EPA) regulations implementing the Asbestos Hazard Emergency Response Act of 1987 (AHERA) requires annual notification of parent and employee organizations of the availability of the district’s asbestos management plan. The management plan identifies the location of asbestos containing building materials, its condition and friability and the response action to be taken. The management plan is on file in the Central office. It will be made available for inspection within five (5) working days of receipt of a written request.

    Pesticide Notice:
    New York State Education Law Section 409-H, effective July 1, 2001, requires all public and nonpublic elementary and secondary schools to provide written notification to all persons in parental relation, faculty, and staff, regarding the potential use of pesticides periodically throughout the school year. The Salamanca City Central School District is required to maintain a list of persons in parental relation, faculty, and staff, who wish to receive 48-hour prior written notification of certain pesticide applications. The following pesticide applications are not subject to prior notification requirements:
    • A school remains unoccupied for a continuous 72-hours following an application of Anti-microbial products
    • Nonvolatile rodenticides in tamper-resistant bait stations in areas inaccessible to children
    • Nonvolatile insecticidal baits in tamper-resistant bait stations in areas inaccessible to children
    • Silica gels and other nonvolatile ready-to-use pastes, foams or gels in areas inaccessible to children
    • Boric acid and disodium octaborate tetrahydrate
    • The application of EPA designated biopesticides
    • The application of EPA designated exempt materials under 40CFR152.25
    • The use of aerosol products with a directed spray in containers of 18 fluid ounces or less when used to protect individuals from an imminent threat from stinging and biting insects including venomous spiders, bees, wasps, and hornets
    In the event of an emergency application necessary to protect against an imminent threat to human health, a good faith effort will be made to supply written notification to those on the 48-hour prior notification list.
