STUDENT WORKING PAPERS

Young people between the ages of 11 and 17 who plan to work part-time or full-time are required to have working papers.

Applications for working papers may be obtained from the Salamanca High School Office.

You will need to bring:

* Birth Certificate, Driver's License or Passport
* Proof of physical within a year from physician
* Proof of Residency
A parent needs to sign the Application for Employment and the child needs to sign the working paper card in front of the clerk.

Below, you will find electronic documents that you can print out and complete.

The process:

Student picks up or prints the working paper application, fills out and signs along with parent or guardian

Student returns signed completed application to the nurses office for physical verification.

School nurse will sign off and return verified application to the high school office

High school office will complete the working card, and student will have to sign working card in front of clerk.
