SCCSD will host COVID booster clinics in coordination with the Cattaraugus County Health Department.
January 27, 2022 from 4:00-6:00pm in the Seneca Intermediate Multipurpose Room
Booster Janssen Vaccine Clinic:
This vaccine clinic is only intended for individuals who have received 1 dose of Janssen (Johnson and Johnson) vaccine with the initial dose being 2 months prior.
Please bring the following with you to the appointment: MANDATORY
a. Proof that you are eligible for the vaccine (professional license or ID, birth certificate/passport/drivers license (age) etc.)
b. Your registration confirmation form (from email or a printout of the last page after you hit REGISTER)

Moderna Booster:
This vaccine clinic is only intended for individuals who have received 2 doses of mRNA vaccine and meet the following criteria provided by the CDC:
The CDC has recommended Pfizer boosters vaccines for any individual over the age of 16 who received a second dose 6 months ago.
Please bring the following with you to the appointment: MANDATORY
a. Proof that you are eligible for the vaccine (professional license or ID, birth certificate/passport/drivers license (age) etc.)
b. Your registration confirmation form (from email or a printout of the last page after you hit REGISTER)
